:: Myopia 

Myopia (short-sightedness) is a vision defect due to a refractory error. In short-sighted people the eye is longer in proportion to the eye’s refractive ability therefore the picture of a remote object does not fall on the retina, whereas the light rays that arrive from remote objects in normal conditions (emmetropia) fall directly on the retina. In the myopic eye the rays fall in front of the retina, producing blurred images. A shortsighted person has difficulty in seeing objects in the distance, however has no difficulty in seeing objects nearer to the eye. Since the main cause of myopia is a longer eyeball compared to normal, it is probable that this condition may worsen with age. Onset of myopia is frequent after 10 years of age, continues through puberty, and evolves to be more evident in later years. The causes are not clear but it is probable that hereditary, constitutional and environmental factors play a major role.

The first signs that demonstrate that a person may have myopia are squinting of the eyes in order to focus objects in the distance. Visual acuity is measured by asking the person to “read” signs and symbols on acuity charts called octotypes, which are scaled in dyoptres (normal visual acuity is 11/10). The traditional solution to contrast this defect is to prescribe corrective spectacle lenses or contact lenses. The lenses for myopic eyes are biconcave or diverging so that the light rays that reach the eye from remote objects fall on the retina and produce a defined image. Often contact lenses are preferred for aesthetic and/or practical reasons. It is now possible to treat myopia via laser surgery; however highly specialised personnel must evaluate the conditions of the eyes since not all myopic patients can undego laser surgery.

DEGENERAZIONE MACULARE LEGATA ALL'ETA

La degenerazione maculare legata all'età è in gran parte dovuto a un processo di invecchiamento, anche se non si esclude una predisposizione genetica.
La malattia colpisce più frequentemente persone dopo i 50 anni di età ed aumenta con frequenza maggiore nelle fasce di età più avanzate, l’ incremento della vita media è probabilmente una delle cause dell’aumento delle diagnosi.
Forme di degenerazione maculare legata all'età
Esistono due forme di degenerazione maculare legata all'età, una "secca" ed una "umida".
Nella forma secca (90% dei casi), depositi puntiformi giallastri chiamati "drusen" sono presenti nella regione maculare e uno degli strati della retina (l'epitelio pigmentato retinico) va incontro ad una progressiva atrofia con conseguente perdita dei fotorecettori.
In questa forma la vista è di solito abbastanza ben preservata.
Nella forma "umida" (10% dei casi), vasi sanguigni anormali crescono sotto la retina nello spazio sottoretinico e lasciano diffondere liquidi dal sngue, per questa ragione è detta umida.

INDICATIONS:

Myopia (short-sightedness) is a refractory defect, not a pathology. However an anatomical or functional defect can be the cause of various retinal pathologies:

- Posterior outpouching of the globe with thinning and rarity of retinal pigment epithelium. Rarity of the RPE is expression of fatigued cellular activity: it can be caused by excessive activity in YOUNG people and the natural functional aging of the tissue in the ELDERLY. Posterior outpouching of the globe causes modification of retinal metabolism.

- Atrophy of the choriocapillaris layer: index of tissue damage in myopia (extension of damaged area and partial or total loss of vascular elements).

The refractory surgery technique to correct myopia was an achievement reached during the last decade. To be able to abandon eyeglasses or contact lenses, for professional, aesthetic or practical reasons is an advantage gained by all shortsighted people. However retinal problems remain.
